After opening the above link follow these directions for filling out this form.
1. On the tool bar at the top of the page click on the "SIGN" button.
2. You must save a copy of the document on your computer to be able to write on it.
3. Open the saved document and click on "SIGN", click on the "Add Text" button, click on the form and
fill in the necessary boxes.
4. Click on the "Done Signing" button. This will save the changes that you have made. Don’t
send the document via Adobe. Just email us the finished form with a copy of your photo ID and credit
card. We need these copies to complete your application.
